The Solution

This solution addressed all the challenges faced. The client had created a subscription-based model that allowed patients to choose from different care plans on a monthly and annual basis. LogicRain to cater these needs built the below modules,

Pharmacist & Patient Coordinator Portal allowed pharmacists and patient coordinators to manage patient data, medication schedules, and other important information.

Health Cloud + Experience cloud integration allowed the team to leverage Salesforce’s powerful features to create a secure and scalable platform.

Schedule tab (calendar view) of all the patients developed a schedule tab that provided a calendar view of all the patients, making it easier for patient managers to track patient schedules.

Reader tabs patient’s medicine schedules included reader tabs that allowed patient coordinators to track patient medication schedules and activities.

Email automation about patients’ activity and patients’ readings was critical, the system automatically sent email notifications to patient managers and doctors.

The Billing module generated a code to define the revenue generated.

Medication adherence module included a medication adherence module that required users to follow the schedule properly to take pills at the desired time. Patient coordinators tracked patient activities, and users received SMS reminders for pill reminders.

Session logs patient-wise, note-taking had session logs for patient coordinator interactions, which allowed patient managers to track patient progress and note-taking.

The Result

The solution developed by LogicRain Technologies allowed the healthcare company to provide high-quality care to their patients while also generating revenue.

The subscription-based model provided patients with affordable and customizable care plans, while the medication adherence module ensured that patients followed their medication schedules properly.

The solution provided patient managers with the tools they needed to track patient progress and note-taking, while the email automation system ensured that patient managers were notified
